Making an appointments in advance can help us to prepare for your visit, and to reduce waiting time for our patients. Our appointment line number is 763-520-7840.
When calling for your appointment please tell us the reason for your visit. Appointments of an urgent nature will be scheduled as soon as possible. For routine issues, please call us as far in advance as possible, so we can schedule an appointment time that is convenient for you.
If you are unable to keep a scheduled appointment please notify us at least 24 hours in advance.
Please remember that our physicians are sometimes called upon to deal with traumatic injuries on an emergency basis. This may cause unforeseen delays in our appointment schedule. Thank you for understanding.

Co-payment if required by your insurance. Payment is due at the time of service. We accept Visa, Mastercard, American Express and Discover credit cards for your convenience.
We require all insurance cards to be presented at every visit. Your cooperation in complying with this policy will help assure accurate and expedient processing for your claims in this time of frequently changing insurance.
If another physician is referring you to us we will need the physician&s name and address. Insurance plans often require specific referral forms from your primary care provider. Please consult with your insurance company and provide us the necessary referral information.
For work related injuries and motor vehicle accidents we will need your insurance company name and address, a case number or claim number and the date of your injury or accident.
