When you receive services from Oakdale Ear Nose & Throat Clinic you assume the financial obligation of payment for the services rendered.
Health Plans & Insurance
For your convenience we will file claims for your health care directly with insurance carriers. Your insurance will determine whether services are covered. You are ultimately responsible for any costs not covered by your health plan.
Please inform us of changes in your name, address, phone number, employer, health plan, group or policy number to make insurance filing more efficient.
Copayments, Credit & Billing
Copayments are due at the time of service. Patients who have over due accounts, are not local residents, or are new patients without insurance will be expected to pay in full at the time of the visit. You will be billed for deductibles, coinsurance and any non-covered services after we have received notification from your health plan.
We accept Visa, MasterCard, American Express and Discover credit cards for your convenience.
If you have questions about your bill please call our Patient Account Service Department at
763-520-7860. This number will also be listed on your statement each month.

MEDICAL RECORDS

Each patient seen by Oakdale Ear Nose & Throat Clinic has a medical record. This record is kept at our Robbinsdale office. We consider all information in your file confidential, and we are committed to protecting your privacy.
No information from your chart will be released without your written consent. If the request is for a minor the parent or guardian must sign the release form. If the patient is 18 or older they must sign the release form.
If another clinic or doctor has sent your medical information to us, we cannot release this information. You must contact the original source and request that information.
Please allow a minimum of one week for us to process your records and send them to you or another doctor&s office.
