To make an appointment with any of our physicians, you may call 763-233-5799 or register on our patient portal and request an appointment. The patient portal is a secure web portal so you can be specific on who you would like to see for what problem and where you would like to see them.

Your Visit:

Please come prepared with your insurance information, a picture ID, and list of medications you are currently taking. If you wish, you can pre register from the comfort of your home or office by going to the patient portal and completing the Patient Registration form and Health History. Then when you come for your appointment, the forms will be ready for your signature. Please make sure you bring you insurance card and picture ID.
