Medical Records

Each patient seen by Oakdale ENT clinic has a medical record. This record is kept at our Robbinsdale office. We consider all information in your file confidential, and we are committed to protecting your privacy.

No information from you chart will be released without your written consent. If the request is for a minor the parent or guardian must sign the release form. If the patient is 18 or older they must sign the release form.

If another clinic or doctor has sent your medical information to us, we cannot release this information. You must contact the original source and request that information.

Please allow a minimum of one week for us to process your records and send them to you or another doctor’s office.
